The Oncology  Registered Nurse Navigator (ONN) is a RN who serves as a key member of the multidisciplinary team, providing clinical guidance, education, and support to oncology patients and their families within the ambulatory care setting. The Oncology RN will work with the Clinical Operations, Nursing staff and Physicians to ensure clinical goals are met through determination of work load priorities to meet the clinic’s administrative, operational and support requirements.  Excellent communication (written and verbal) skills. The ONN facilitates timely access to care, coordinates appointments and diagnostic services, and ensures clear communication between providers, patients, and caregivers.  The ONN helps patients overcome the challenges of a complex medical system as it relates to timely oncology care, treatment adherence, patient outcomes, and serving as an advocate, educator, and a resource from diagnosis through survivorship. 

Primary Responsibilities:

  •  Provide care for critically or chronically ill cancer patients
  • Create a supportive environment, educating cancer patients about treatment procedures and particularities of the disease
  • Work with patients who have been diagnosed with life – threatening conditions to provide a stable environment to help patients and their families with work through the treatment process and to cope with potentially fatal outcomes
  • Ensure strict compliance with federal and state regulations and company policies, procedures and processes
  • Ensure patient care is delivered appropriately and consistent with best – practices and industry standard measures, i.e. QOPI
  • Works with less structured, more complex issues
  • Serves as a resource for others
  • Assesses and interprets patient needs and requirements
  • Identifies solutions to non-standard requests and problems
  • Solves moderately complex problems and/or conducts moderately complex analyses
  • Works with minimal guidance; seeks guidance on only the most complex tasks
  • Translates concepts into practice
  • Provides explanations and information to others on difficult issues
  • Coaches, provides feedback, and guides others
  • Collects subjective and objective health status information from multiple sources such as; patients, caregivers, members of the health care team, documented records and other relevant sources.
  • Prioritizes activities based on the patient’s immediate health need or the nurse’s judgment of anticipated patient needs.
  • Documents the information and data collected in a retrievable understandable and readable format.
  • Employs nursing knowledge and evidence nursing practice in caring for patients in the office and over the phone
  • Includes the patient, appropriate caregivers, and other health professionals in making shared decisions about plans and strategies.
  • Considers the patient or population needs in terms of age, gender, race, cultural values and practices, ethical and legal considerations and environmental factors as well as the anticipated risks and benefits of interventions for individualized plan development.
  • Implements the plan of care in partnership with the patient, caregivers and other healthcare professionals in a timely manner.
  • Performs interventions supported by evidence- based ambulatory care nursing practice during clinical visits, telephone encounters and electronic communications according to regulations, standards and organizational policy and procedure.
  • Provides age appropriate, population specific care in a compassionate, caring, culturally and ethically sensitive manner.
  • Delegates appropriate nursing interventions to clinical team members based on competency and organizational polices.
  • Provides relevant information to individuals within and/or outside of Crystal Run Healthcare when the patient’s care is transferred between and among different specialties.

Required Qualifications:

  • Graduate of an accredited School of Nursing
  • Current NYS License as a Registered Professional Nurse

Preferred Qualifications:

  • OCN certification
  • Supervisory experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $34.23 to $61.15 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
