As a Social Worker you will be responsible for validating referrals, collaborating with teams to assess member needs, and identify gaps in care. They will also document findings and analyze data to uncover barriers to care.

You’ll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. 

Primary Responsibilities:

  • Referral and Assessment 
    • Receive and validate referrals for members’ health or psychosocial needs
    • Follow protocols to respond appropriately to referrals
    • Review diagnoses and case files to understand referral context
    • Collaborate with interdisciplinary teams to assess member needs
    • Identify gaps in care and seek additional information as needed
    • Analyze data to uncover root causes and barriers
    • Document findings per organizational standards
  • Motivational Interviewing 
    • Build therapeutic relationships using appropriate communication
    • Use open-ended questions, active listening, and empathy
    • Confirm referral needs with members and educate them on goals
    • Guide conversations to align with assessment objectives 
  • Identifying Support Opportunities 
    • Match community/government resources to member needs
    • Refer members to appropriate experts when outside social work scope
    • Respect member diversity and advocate for their perspectives
    • Prioritize urgent needs and navigate systems for resources
    • Collaborate with teams to develop support plans 
  • Implementing 
    • Coordinate and deliver services at client and system levels
    • Provide resources and guidance to secure support
    • Adapt strategies to overcome service access challenges
    • Collaborate with teams to meet member needs
    • Adjust and document care plans as needed 
  • Monitoring and Evaluating 
    • Maintain contact with providers and members
    • Evaluate intervention effectiveness and revise plans
    • Update teams and advocate for member needs
    • Close cases when goals are met, or no further action is possible
    • Document outcomes in appropriate systems

Required Qualifications:

  • Bachelor’s degree in Social Work
  • 2+ years of Behavioral Health / Substance Use Disorder experience 
  • 2+ years of experience in the health care industry including clinical experience, Medicare / Medicaid, insurance, health promotion, and operations
  • Intermediate level of proficiency using a PC in a Windows environment, including Microsoft Word and Excel

Preferred Qualifications:

  • Master’s degree in Social Work
  • Medicaid and / or Medicare experience
  • Proven excellent customer service orientation
  • Proven to remains optimistic, upbeat, and enthusiastic in times of challenge and change
  • Proven self-directed worker and ability to work independently
  • Proven highly organized
  • Proven excellent written and verbal skills
  • Proven excellent interpersonal and problem-solving skills
  • Proven Team Player

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $23.41 to $41.83 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
