This position is full-time Monday-Friday 9am-6pm PST. Employees are required to work our normal business hours. It may be necessary, given the business need, to work occasional overtime or weekends. 

You’ll enjoy the flexibility to telecommute* from anywhere within the U.S. as you take on some tough challenges.

 

Primary Responsibilities: 

  • Administrative and Communication Support 
  • Regularly check shared team email inboxes for messages from patients, care coordinators, and leadership, and route them appropriately. 
  • Monitor alerts in the Vivify app and follow established workflows to assign tasks to clinicians. 
  • Keep team spreadsheets on Microsoft Teams current, including creating new monthly tabs and updating entries as needed. 
  • Learn and use internal systems to support daily operations. 
  • Take clear and organized notes during team meetings to support follow-up and documentation. 
  • Assist clinicians by reviewing and updating tracking spreadsheets, including entering basic statistics or data points.

 

Referral and Intake Support 

  • Receive and review incoming referrals for patients with health or social needs. 
  • Follow clear procedures to ensure referrals are handled correctly. 
  • Read through patient information to understand the reason for the referral. 
  • Work with nurses, social workers, and other team members to help assess patient needs. 
  • Help identify missing information and request it when needed. 
  • Organize and summarize basic data to support care planning. 
  • Keep accurate records following company guidelines.

 

Patient Communication and Support 

  • Build positive, respectful relationships with patients through clear and caring communication. 
  • Use active listening and ask thoughtful questions to understand patient concerns. 
  • Confirm what services the patient needs and explain next steps. 
  • Help guide conversations to gather helpful information for the care team. 

 

Connecting Patients to Resources 

  • Help match patients with internal programs that meet their needs. 
  • Respect each patient’s background and advocate for their preferences. 
  • Recognize urgent needs and help patients access support quickly. 

 

Coordinating Services 

  • Help arrange follow up to make sure patients receive support. 
  • Provide patients with information and tools to access care. 
  • Adjust plans when challenges come up and find new ways to help. 
  • Communicate with the team to make sure everyone is aligned. 

 

Tracking Progress 

  • Record outcomes and updates in the system accurately.

Required Qualifications: 

  • High School Diploma / GED OR equivalent work experience 
  • Must be 18 years of age OR older 
  • 2 + years of experience working in an office setting where daily responsibilities involved frequent use of the telephone and computer as primary tools. 
  • 2+ years of experience analyzing and solving customer problems 
  • Experience working with medical terminology 
  • Experience working in the health care industry 
  • Ability to work fulltime, Monday-Friday 9am-6pm PST. Employees are required to work our normal business hours. It may be necessary, given the business need, to work occasional overtime or weekends. 

 

Preferred Qualifications:

  • Experience working with health care insurance 
  • Experience working with Medicare and/or Medicaid Services 
  • Experience in a hospital, physician’s office, or medical clinic setting 
  • Clerical or administrative support background or experience working in a call center environment 

 

Telecommuting Requirements: 

  • Ability to keep all company sensitive documents secure (if applicable) 
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy. 
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $20.00 – $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
