We seek a Home Infusion Pharmacist to perform professional duties and responsibilities associated with the processing of prescriptions onsite at our Home Infusion Pharmacy in St. Louis, MO. This role is a critical function of the pharmacy to ensure patient safety and compliance and is responsible for: dispensing IG/Chronic medications, communicating with patients and providers, evaluating clinical appropriateness and accuracy of all medications and supplies, developing SOC patient care plans, and monitoring side effects and interactions. Work schedule will be Monday – Friday, 8:30am – 5:00pm CT; will be required to participate in the On-call Pharmacist rotation every other week.
Primary Responsibilities:
- Dispenses unit doses, specialty pharmaceuticals, injectables, IV, enteral and related supplies following state and federal regulations and company policies and procedures
- Processes, packs and ships outgoing pharmacy orders, ensuring drug, related supplies, and patient collateral (education materials) are included
- Performs drug/supply ordering and inventory/warehouse duties, including proper storage of drugs and supplies and appropriate refrigeration of perishable product, monitors and adjust/calibrate pharmacy equipment, fill dispensing units
- Monitors and verifies all prescription orders upon creation and dispensing; calibrates dosing pumps as applicable
- Interprets provider prescription and creates order; accesses, inputs and retrieves prescription and medical/lab information from computer system; verifies whether order/drug is reimbursable/covered
- Contacts members/patients to validate data, resolve rejected claims, request payment information, establish priority of order or delivery details, consult on medication issues, and set up treatment in on site facilities
- Preps and scans documents, verifies member/patient data, submits prescription orders in computer system
- Reviews medication orders and evaluates appropriateness and completeness of orders with consideration given to the special needs and behaviors of the specific age group served
- Verifies all prescriptions not directly received from the prescriber or physician’s agent
- Assesses the patient at the initiation of drug therapy, develops a patient care plan and drug monitoring regimen, and monitors the patient according to the care plan throughout the course of therapy
- Communicates with the patient, physician, nursing staff, other pharmacists, and other members of the health care team regarding patient status, response to therapy, and clinical objectives to ensure clinical outcomes are met
- Where applicable, compounds intravenous and specialty pharmaceuticals in an aseptic fashion according to established State Board of Pharmacy, Federal, USP797 sterile guidelines and company policy; is knowledgeable of the proper use and handling of both sterile and hazardous medications; compounds and handles sterile and hazardous medications in accordance with established guidelines and regulations
- Ensures compliance with all FDA, DEA, State Board of Pharmacy, OSHA, and other regulatory standards; follows all department policies and procedures
- Provides indirect oversight of pharmacy technicians and pharmacy support staff
- Provides oversight of the safety, appropriateness, and effectiveness of the pharmaceutical care provided to patients (consumers) as it relates to specialty products dispensed

Required Qualifications:
- Bachelor’s degree in Pharmacy or PharmD
- Current pharmacist’s license in the state of Missouri
- 2+ years of pharmacy practice experience
- Demonstrated experience communicating and working effectively with a multidisciplinary team
- Proven drug product and clinical pharmacy knowledge
- Intermediate level of proficiency with computers, Microsoft Outlook (ability to manage emails and calendar), and Microsoft Excel (ability to enter, sort, filter and interpret data)
- Ability to take calls after hours, weekends and holidays as scheduled
- Available to participate in the on-call rotation based on business needs
Preferred Qualifications:
- 1+ years of home infusion or home care experience
- Knowledgeable in various infusion related therapies and ability to work through the complexity of therapy-related orders
- Sterile compounding experience
- Experience developing or operationalizing medication clinical guidelines and/or formulary development
- Experience working within a managed healthcare system
- Knowledge of WellSky CPR+ Dispensing Platform
The hourly range for this role is $43.17 to $84.95 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
