Positions in this function receives direction operationally from RN Supervisor of the Anticoagulation Clinic. Works collaboratively with Coumadin Clinic Team, Cardiology RN Manager, Medical Director of the Anticoagulation Clinic, OptumCare Primary Care and Network Physicians. Facilitates appropriate medication management in collaboration with managing provider to maintain excellent customer satisfaction while achieving department specific goals. Works telephonically with patients and providers to manage patient anticoagulation therapy.

Primary Responsibilities:
- Coordination and quality of care of Anticoagulation Clinic patients
- Provides telephonic patient care and management of anticoagulation therapy patients enrolled in the Anticoagulation Clinic
- Evaluates INR results, identify risk factors, and facilitate brief interruptions in therapy across various disciplines and venues
- Collaborates with OptumCare Primary Care and Network Physicians to ensure appropriate and safe anticoagulation therapy management
- Participates in the development and implementation of patient centered care plans
- Ensures compliance with all OptumCare policies and procedures, state and federal regulations, and standards of practice

Required Qualifications:
- High School diploma and/or equivalent
- Registered Nurse with active, unrestricted license in the State of Nevada
- 2+ years of varied clinical and/or case management experience
- Advanced knowledge of anticoagulant therapy including management, indications, and INR responses
- Understand spectrum telephonic care delivery system
- Proficiently demonstrate the skills of RN Case Management and excellent clinical skills with particular emphasis on anticoagulation therapies
- Demonstrate initiative toward problem solving
- Proven telephonic assessment of patient needs to coordinate appropriate level of care
- Demonstrated ability to effectively communicate with professionals, patients, and patient caregivers
- Proven integrate guidelines and tools to negotiate most effective plan of care
- Proven hand-off patient to primary venue so long-term plan can be implemented

- Bachelor’s degree
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
