This position is full time Monday – Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:30 am – 5:00 pm local time. It may be necessary, given the business need, to work occasional overtime.

 

We offer 1 week of training after 1st week they will move to their normal schedule. The hours during training will be 8:30 am to 5:00 pm, Monday – Friday

Primary Responsibilities:

  • Telephonic outreach to members to complete post ER visit assessment. 
  • Completing telephonic assessment with members
  • Facilitating follow up for the member
  • Other duties as assigned

Required Qualifications:

  • High School Diploma / GED OR equivalent work experience
  • Must be 18 years of age OR older
  • 2+ years of experience working in a physician’s office, SNF, ALF, OR outpatient clinic with patient and medical team communication and collaboration
  • Experience working in cross functional, inter and multi-disciplinary teams
  • Telephonic and written communication skills
  • Experience and proficiency with EMRs
  • Knowledge of medical terminology
  • Ability to to work any of our 8-hour shift schedules during our normal business hours of 8:30 am – 5:00 pm local time. It may be necessary, given the business need, to work occasional overtime.

 

Preferred Qualifications:

  • Experience in Microsoft Word (edit, create & save documents), Microsoft Excel (adding, sorting & filtering data) and Microsoft Outlook (email, folders, attachments and calendaring)

 

Telecommuting Requirements:

  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
